A Person-Centered Plan is important to help you respect yourself and others, live safely, eat healthily and be active within your immediate and wider community. We will make this plan with you based on your own aims and what is important to you so that you can build your skills and get the support you need, when you need it. We’ll also review how things are going and work with you continually to update your goals and make good choices

We work with a person-centred approach to fit our service users around them. We will talk and listen carefully to understand what your needs are and how you would like us to deliver your care. We will also ensure that we support you in making informed choices and decisions that will assist towards your progress of gaining independence.

This is about how we act on what you tell us. Feedback is how we shape and develop our services to make them relevant to the people we serve. We collect feedback from the person receiving care, their loved ones, doctors, and other care professionals.

We ensure that the necessary procedures are in place to provide a safe environment to support and protect the residents from risk and harm.
